Bu çalışmada Almanya’nın Hamm şehrinde yaşayan, 50 yaş üstü Türk asıllı yaşlıların sağlık ve evde bakım hizmetlerine erişimindeki mevcut durum ve yaşanan sorunların tespitine yönelik bir anket ve saha çalışması gerçekleştirilmiştir. Yapılan anket ve saha çalışmaları sonucunda elde edilen bulgulara dayanılarak çeşitli çözüm önerileri sunulmuştur. Yaşanan sorunlar önem sırasına göre, Türk asıllı yaşlıların verilen hizmetlerden haberdar olmaması, sunulan hizmetlerden yararlanmada yaşanan çekince ve belediyenin Türk asıllı yaşlılara yeterince erişemiyor olması ve son olarak da dil ve kültür unsurları şeklinde özetlenebilir. Bu sorunların çözümü noktasında, belediyenin yaşlılara, engellilere ve sürekli bakıma ihtiyaç duyan kişilere sunulan hizmetlere ilişkin etkin bir iletişim sistemi geliştirmesi ve bunu ikinci kuşağı da içine alacak şekilde genişletmesi önerilmektedir. Hamm Belediyesi ayrıca Türk asıllı yaşlılar için ikili toplum yapısı ve geciken entegrasyon dolayısıyla duyduğu kaygıyı ortadan kaldırmak amacıyla etkin bir veri tabanı çalışması yapmalıdır. Yapılan veri tabanı çalışması sonuçlarına göre (gizli olabilecek bilgiler paylaşılmaksızın) tespit edilen yaşlı, engelli veya sürekli bakıma ihtiyaç duyan kişilerin bulunduğu aileler, belediyenin ilgili birimlerinin Türk ve Alman asıllı çalışanları ve ilgili sivil toplum kuruluşlarının organizasyonu ile periyodik olarak ziyaret edilmelidir. Ziyaret edilen ekipte Türk asıllı bir çalışanın bulunması hem iletişim açısından hem de ailenin kendisini rahat ve güvende hissetmesi açısından büyük önem taşımaktadır. 

In this study, a survey and field studies were conducted to determine the current situation and the problems experienced in the access of Turkish-origin elderly people over 50 years of age living in Hamm, Germany to healthcare services. Various suggestions were presented based on the data obtained as a result of these studies. The problems experienced can be given as the importance of the elderly people of Turkish origin not being aware of the services provided, the hesitation experienced in benefiting from the services provided, the inability of the municipality to reach the elderly people of Turkish origin, and finally, the influence of language and cultural factors. To solve these problems, the municipality should develop an effective notification system regarding the services offered to the elderly, disabled people, and people in need of continuous care and expand this to include the second generation. The municipality should carry out an effective database study for elderly people of Turkish origin to eliminate the concerns about the dual social structure and delayed integration. According to the results of the database study (without sharing information that may be confidential), families with elderly, disabled, or in need of permanent care should be visited with the Turkish and German employees of the relevant units of the municipality and the organization of the relevant non-governmental organizations. The presence of an employee of Turkish origin in the team visited is of great importance in terms of communication and for the family to feel comfortable and safe.
