Sağlık Hizmetlerine Duyulan Güvenin Analizi

Bu kesitsel çalışmada, İstanbul İli Anadolu yakasında sağlık hizmeti alan bireylerin sağlık sistemine duydukları güveni; sağlık profesyonellerine, sağlık finansmanını karşılayan ve hizmeti sunan kurumlara duydukları güven boyutlarıyla tespit etmek amaçlanmıştır. Kesitsel olarak tasarlanan araştırmaya, son bir yıl içerisinde ayakta ve yatarak sağlık hizmeti alan 406 kişi (K=292, E=114) katılmıştır. Veri toplama formu olarak 17 maddeden oluşan “Sağlık Hizmetleri Sistemi Çok Boyutlu Güven Ölçeği” (X ̅=56.09; SS=12,44) kullanılmıştır. Üç alt boyuttan oluşan ölçeğin analizinde; sağlık profesyonellerine (X ̅=34,87; SS=7,98), maliyeti karşılayan kurumlara (X ̅=12,68; SS=3,50) ve sağlık kurumlarına (X ̅=9,36; SS=2,67) duyulan güvenin ortalamanın üstünde olduğu tespit edilmiştir. Ölçek toplam ve alt boyut puanlarında cinsiyet, yaş, eğitim durumu gibi değişkenlere göre anlamlı fark bulunmamıştır. Sağlık profesyonellerine ve sağlık kurumuna duyulan güven boyutunda özel hastanelerin devlet hastanelerine göre puanlarının anlamlı düzeyde yüksek olduğu görülmektedir. Sağlık profesyonellerine güveniyorum diyen katılımcıların oranı eczacı (%75), hekim (%70), diş hekimi (%63), hemşire (%60), fizyoterapist (%55) ve psikolog (%49) olarak sıralanabilir Gelirin, giderine eşit ve fazla olduğunu beyan eden katılımcılar sağlık profesyonellerine daha fazla güven duymaktadırlar. Özel hastanede tedavi gören katılımcıların sağlık profesyonellerine ve sağlık sistemine duydukları güven, devlet hastanesinde tedavi görenlere göre yüksektir. Sonuç olarak katılımcıların sağlık sistemine duydukları güven düzeyi ortalamanın üzerinde olmakla birlikte aynı ölçekle yapılan diğer çalışmalar ile karşılaştığında yıllara göre bir düşüş olduğu göze çarpmaktadır. Güveni etkileyen faktörlerin tespit edilerek iyileştirme çalışmaları planlanması önerilmektedir.

This cross-sectional study was carried out to evaluate individuals’ trust in healthcare systems towards healthcare professionals, insurers (institution of health financing) and health institutions in the Anatolian side of Istanbul. The number of participants was 406 (F=292, M=114), receiving outpatient and inpatient health services last year. The Multidimensional Trust in Health Care Systems Scale (X ̅=56.09; SD=12.44) was used as a data collection form consisting of 17 items. The mean scores of health professionals (X ̅=34.87; SD=7.98), health financing institutions (X ̅=12.68; SD=3.50), and health institutions (X ̅=9.36; SD=2.67) were higher in all sub-dimensions. There are no significant differences in the total and sub-dimension scores of the scale according to variables such as gender, age, and educational status. The mean scores of private hospitals are significantly higher than public hospitals in terms of trust in health professionals and health institutions. The percentage of participants’ trust toward health professionals can be listed by the percentages of the following: pharmacists (75%), physicians (70%), dentists (63%), nurses (60%), physiotherapists (55%), and psychologists (49%). Participants with an income higher than or equal to their expenses have more confidence in health professionals. The participants’ trust treated in a private hospital in health professionals and the health system is higher than those treated in a public hospital. As a result, although the participants’ trust in the health system is above the average, there has been a decrease in this trust over the years compared to previous studies conducted with the same scale. It is recommended to determine the factors affecting trust and plan improvement studies.
