Bu çalışmada, nadir hastalıklar ve yetim ilaçlarla ilgili sağlık politikası kararlarının objektif ve rasyonel bir şekilde verilmesi için en etkili ve öncelikli politika belirleme faktörlerinin, DEMATEL yöntemiyle tespit edilmesi amaçlanmıştır. Literatür taramasına göre sağlık politikası karar vericilerinin kullanabileceği; tedavi/ilaç güvenliği, etkilenen nüfusun büyüklüğü, alternatif tedavinin varlığı, hastalığın şiddeti, bütçe uygunluğu, toplumsal tercihler, fon sağlayıcıların tercihleri, politik çıkarlar ve maliyet-etkililik olmak üzere 9 politika belirleme faktörü tespit edilmiştir. Faktörlerin birbirlerine olan etki düzeylerini belirlemek ve öncelikli olarak sıralayabilmek için DEMATEL yöntemi kullanılmıştır. DEMATEL sonuçlarına göre, nadir hastalıklar ve yetim ilaçlarla ilgili sağlık politikası kararlarının objektif ve rasyonel bir şekilde verilmesi için en etkili ve öncelikli politika belirleme faktörleri sırasıyla %12,40 ağırlık ile “Hastalığın Şiddeti”; %12,19 ağırlık ile “Maliyet-Etkililik”; %12,17 ağırlık ile “Alternatif Tedavinin Varlığı” olarak tespit edilmiştir. En önemsiz politika belirleme faktörü ise %8,94 ağırlık ile “Fon Sağlayıcıların Tercihleri” olarak bulunmuştur. Nadir hastalıklar ve yetim ilaçlar için sağlık politikası oluşturmanın karmaşıklığını daha etkin ve sürdürülebilir şekilde değerlendirmek çok yönlü ve çözüm odaklı bir yaklaşımı gerektirmektedir. Kapsamlı hasta kayıt sistemleri ve veri tabanları ile hastalık yüküne dair doğru ve güncel bilgiler toplanarak, politika yapıcılara rehberlik edecek şekilde karar destek sistemleri oluşturulabilir. Kamu-özel iş birliğinin teşviği, ilaç geliştirme sürecinde ar-ge maliyetlerini azaltarak tedaviye erişimi artırabilir. Üniversiteler, araştırma merkezleri ve ilaç firmaları gibi sürecin paydaş grupları arasında iş birliğini artırıcı stratejiler geliştirilmeli; gen terapisi gibi alternatif tedavi projelerine finansal destek sağlanmalıdır.

This study aims to identify the most effective and prioritized policy-making factors for making objective and rational health policy decisions on rare diseases and orphan drugs by using DEMATEL method. According to the literature review, 9 policy-making factors were identified as treatment/drug safety, size of the affected population, availability of alternative treatment, severity of the disease, budget suitability, social preferences, preferences of funders, political interests and cost-effectiveness. DEMATEL method was used to determine the level of influence of the factors on each other and to prioritise them. According to the DEMATEL results, the most effective and prioritised policy-making factors for making objective and rational health policy decisions on rare diseases and orphan drugs were found to be ‘Severity of Disease’ with a weight of 12.40%, ‘Cost-Effectiveness’ with a weight of 12.19%, and ‘Availability of Alternative Treatment’ with a weight of 12.17%, respectively. The least important policy determination factor was found to be ‘Preferences of Funders’ with a weight of 8.94%. A more effective and sustainable approach to assessing the complexity of health policymaking for rare diseases and orphan drugs is required, and this will necessitate a multifaceted and solution-oriented methodology. The establishment of comprehensive patient registries and databases can facilitate the collection of accurate and up-to-date information on the burden of disease, thereby enabling the creation of decision support systems to guide policymakers. The formation of public-private partnerships can increase access to treatment by reducing the costs associated with the drug development process. Strategies should be developed to enhance collaboration among stakeholder groups, including universities, research centres and pharmaceutical companies, and financial support should be provided for alternative treatment projects, such as gene therapy.
